The Karen P. Nakon Breast Cancer Foundation was founded on February 22, 2003, Karen’s 38th birthday and the day she died, to honor her graceful and courageous five-year battle against breast cancer. One of the major objectives of The Foundation is to help families endure the staggering costs associated with living with, battling and surviving this disease. Learn more about our previous grant recipients and their families CLICK HERE TO DOWNLOAD APPLICATIONGeneral InformationWho Is Eligible For Assistance?Any patient or family who has been impacted by breast cancer and is in financial need.Funding is currently limited to the following counties in the State of Ohio: Allen, Ashland, Ashtabula, Auglaze, Belmont, Carroll, Crawford, Columbiana, Coshocton, Cuyahoga, Defiance, Erie, Fulton,Geaugua, Hancock, Hardin, Harrison, Henry, Holmes, Huron, Jefferson, Lake,Logan, Lorain, Lucas, Mahoning, Medina, Mercer, Ottawa,Paulding, Portage, Putnam, Richland, Sandusky, Seneca,Shelby, Stark, Summit, Trumbull, Tuscarawas, Van Wert, Wayne, Williams, Wood, Wyandot. Applicants are eligible for funding once in a twelve month cycle. What Do We Fund?
How Do I Apply?If you wish to receive aid from the Karen P. Nakon Breast Cancer Foundation, you must complete the “Application for Assistance”. We have a link below that will open the application in a PDF for you to print. If you are unable to download the PDF, simply call our office and we will send or email you a copy. Answer all questions fully and have all necessary signatures as part of the application. We cannot process incomplete applications. All signatures must be original and, therefore, stamps, photocopies or initials will not be accepted. Part of the application is “your story” and a description of the areas where you are in need of financial assistance. Since this is the only way that the Board will “meet” you, please be as specific as possible. Know that part of the process of application review the facts as you state them in the application will be verified. Therefore, it is in your best interest to be complete and accurate. When your application is complete, please forward it to The Foundation office at; 35765 Chester Road, Avon, OH 44011. The Executive Committee will review requests. We have no set minimum or maximum limit on the assistance we provide. Each quarter, The Foundations' Board of Directors meet to review grant applications. The funding cycle is as follows:
